The POI (Palliative Outcomes Initiative) is a collaborative programme led by the Auckland specialist hospices to support General Practices and Residential Care Facilities in providing high-quality primary palliative care. The initiative supports healthcare providers with the identification of patients with palliative needs, and provides access to tools, resources, and training to develop tailored care plans, enhancing the overall quality of life for patients. Through this initiative, GPs and care teams gain confidence and skills to manage non-specialist palliative needs, promoting dignity, and quality of life during their patients' palliative journeys.
